Header Menu Suggestion for SQL file

Hi Mike,

I have a suggestion for the SQL files header menu. Adding a button labeled "Create DB Code Notebook" in the header could allow users to create a DB Code notebook directly from the contents of an open SQL file.

This feature would be especially helpful for users who want to view their SQL file contents in a notebook format.

Not going to add this at this stage, will see if more requests for it come in.

I want to keep those icons focused on editor actions (execute code, query plan when it's built). If VSCode ever adds a toolbar then I can add this as there will be more space/room.
