Fixes

Skills no longer gain an extra name heading each time an agent loads and saves them; existing skills are cleaned up on upgrade.
External MCP tool calls in Codex, Claude and OpenCode chats now time out after 10 minutes instead of hanging a chat for hours.
Linking tickets no longer shows a second "created" notification.
The "waiting for approval" hand is now easy to see in the light theme.
Ticket statuses no longer wrap onto two lines in the # mention picker.
