Additional custom form disappears in attendee form setup

When an organizer creates a second custom the standard and custom questions disappear when the user loads the form again.

Replicate:

1. Make an attendee form 1
2. Make an attendee form 2
3. Save the attendee forms
4. Open the attendee forms
5. All or some attendee form elements have disappeared.
