feat(rag): add RetrievalPlan types and plan builder

Introduce structured RetrievalPlan interface replacing the simple
strategy string with granular source selection, HyDE config, memory
type filtering, temporal preferences, graph traversal config, and
RAPTOR layer selection. Add buildDefaultPlan() factory with sensible
defaults per strategy level (none/simple/moderate/complex).
