fix(mars-genesis): fix forge success rate (permissive schemas, code wrapping, mode normalization)

Judge now approves tools on 1st-3rd attempt. 8 tools forged across
3 turns: medical (mars_site_risk_scorer, water_strategy_risk_model,
perchlorate_agriculture_risk_model), engineering (life_support_capacity_model,
water_deficit_model, power_budget_analyzer), agriculture
(water_shortfall_food_risk_projector_v3, perchlorate_agriculture_decision_v1).
Fixed null in tool registry name extraction.
