Getting Started section in README.md does not actually help you get started

Yea, the title is a littl clickbaity - The problem I see in the readme is that it skips the requirement and process to getting VS Code launched and where/how to type the slash commands.

I think there needs to be an indication of some sort that the commands need VS Code, the project needs to be opened to the right place and that the agent window is where you type.

As it stands now, the user will be severely dismayed when the slash commands do not work in their terminal.

Needs to explain the order of operations because You can get slash commands to work as long as you install in the correct location etc..
