In Norway we have discussed the different sections. We have had a national digital discharge summary for decades, and we therefor have a lot of experience on what is perceived as valuable, what is not so valuable and what has caused problems for implementation. One of the most valuable sections in our Discharge summary is "Summary of assessment, including plans for further follow-up" (Norwegian: Vurdering, inkludert planer for videre oppfølging). This section is written as a short conclusion of the stay, and with clear message to the ones responsible for continued care. Receivers of a discharge summary like to read this part first, to rapidly be able to assess if there are actions to be taken and get an idea how the Hospital stay ended up. It is much easier than having to scan various part of the discharge summary to find conclusions or recommendations for follow up. It is time saving for the reader, and it allows the writer of the Discharge summary to focus on a short readable handover statement. To reduce clinician burden we would very much like to promote such a section.

The question is if any of the sections already described fulfill this need. "Care plan and other recommendations after discharge" as an overarching section could serve the need.

We see in the current definition of sectionPlanOfCare with LOINC code 18776-5 | Plan of care note: Plan of care notes (previously defined as "Plan of treatment") contains data that defines pending orders, interventions, encounters, services, and procedures for the patient. The plan includes prospective, unfulfilled, or incomplete orders and requests only. The plan may also contain information about ongoing care of the patient and information regarding goals and clinical reminders.
