docs(inpatient): describe current state only, drop change narrative Remove "previously/was removed/now/recommended/no need to re-type" framing from the inpatient pharmacy pages so they describe only how the features work today: - Ward Medications: drop the old-Drug-Chart section and incidental narrative. - Discharge Medications: drop refresh-in-place note. - Issue Discharge Medicines: factual intro and steps. - EHR Overview: drop Drug Chart removal note; concise section descriptions.
