Renames all references of codeshift to hypermod

[danieldelcore]

• Renames all references of codeshift to hypermod (the new package name / org).
• Removes the test-utils package in favour of utils (they've been merged).
• Creates an alias package for the CLI so existing users of @codeshift/cli will continue to receive updates as per normal.
