Driver fixes from the first live pilot: finds a .cmd wrapper on Windows, reports a missing agent command in a dry run, kills the whole process tree on a timeout, and can log session output with --log.

The driver now stops a hung session (--session-minutes, and the --max-hours deadline always applies). Opening and closing a session no longer counts as progress, so the no-progress stop works. A direct owner request in a session counts as an owner note.
