Notebook changes on disk while opened

Repro:

1. Open Notebook that is checked into git
2. git pull to update the notebook on disk

🐛 => opened notebook is out of date and nothing is mentioned to the user

Moving it here again as extensions need to listen to file change on disk and then update the docu

@rebornix Tricky... How should I update the document? The contract is cell data but there is no straight forward way to update a notebook document t work with cell data.

We need to add new API to allow doing that. This is a good case that vscode.TextEditor is not enough to modify cell content.

Yeah. Or should we, as the core, just ask the extension again for content?
