[Bug] Wrong TAB order on add-breakpoint dialog [sf#316] #4

The TAB order of the fields on the add breakpoint dialog is wrong. Currently it has the following order:
Address, Segment, Subslot, Slot, OK, Cancel
The most logical sequence would be:
Address, Slot, Subslot, Segment.
The bug can be reproduced by trying to add a breakpoint on a MSX Turbo-R, at address 0x9000, slot-3, subslot-0, segment-0
