Patch Changes

• 70997ee: Refresh documentation for the published Effect 4-first 2.x package. Add a runnable
  Bun/npm quick start, clarify explicit Effect 3 preset migration, make current
  agent-skill assets primary, and record the completed 2.0.0 release.

  Add a frozen npm-installed 2.0.0 / Effect 4 DDD example with annotated failures,
  clean repairs, exact JSON diagnostic checks and runtime contracts. Retain the
  legacy npm example separately. No rule behaviour changes.
