Copilot Cloud Agent

I'm getting 401 status code back whenever calling the tool
create_pull_request_with_copilot. It used to work but something changed in the last couple weeks. The permissions for the fine grained PAT are:
Metadata : Read
Content: Read & Write
Pull requests: Read & Write
No documentation is available that describes which permissions are required. Did they change or is this a bug in the remote mcp server?
Has anyone got this working?
There is also this issue: github/github-mcp-server#942
