iOS Mobile 2FA error, returning CryptoTokenKit error -3

In an attempt to login to github web, I'm required to verify 2FA with a number of options I've already configured of which none are working properly including the Mobile two digit verification method.

After clicking "Approve", the app returns the following error:
"The operation couldn't be completed.
(CryptoTokenKit error -3.)"

2FA SMS verification is not working because the verification code never arrives even though my country, Ghana (+233) is supported.

The recovery codes I have saved are also not working.
