User Removed from the organization via "Github System"

One of our members was removed automatically from our organization via the "GitHub System" within the Shift3 organization here: https://github.com/Shift3 we are wondering what triggered this user to be removed from our organization by the "GitHub System"?

We pulled the audit to find this info. We would like users to not be removed in the future and/or an email to be sent out when this occurs.
