Move UUIDs out of propolis-server API endpoints. #62
Description
A single propolis-server process can only ever serve a single instance whereas the current URL scheme implies otherwise with each endpoint prefixed with the instance UUID. It makes sense to keep it around in some form for verification and debug purposes but the ergonomics would be improved without it in the URL.
