Refactor spring-boot-cloudfoundry package structure

The spring-boot-cloudfoundry module currently uses the package org.springframework.boot.cloudfoundry.actuate.autoconfigure. Other modules lead with autoconfigure so we should rename it to org.springframework.boot.cloudfoundry.autoconfigure.actuate.
