put both-type and fetch-type into PDOStatement generic

this allows us to reduce the contained both-type to numeric/assoc no matter which fetch-type is requested

additonally this PR adds support for
defaultFetchMode(QueryReflector::FETCH_TYPE_NUMERIC)
