Add docs regarding BYPASS_TOOL_CONSENT and fix other minor docs issues

Description

In this PR, I am focusing on three things:

1. Addition of documentation related to BYPASS_TOOL_CONSENT env variable. This addition is motivated by the issue created here

2. Fixing diagrams: In the memory_agent.py example, the documentation was still using ASCII diagrams. The change is simply fix this to use Mermaid diagrams to achieve consistency throughout all the docs.

3. Fix formatting issues for bullet points.
