Ignore h4 to h6 headers in document outline (or properly indent them)

Problematic behavior
When using h4 (to h6) tags in a document, the outline seems to consider them as higher level (h1?)

Expected behavior/code
h4 to h6 tags should be ignored in the document outline or properly indented.

Steps to Reproduce
Markdown content:

## Sub-Section 1

### Sub-subsection 1.1

#### sub-sub-subsection 1.1.1

## Sub-Section 2

### Sub-subsection 2.1

#### sub-sub-subsection 2.1.1

From your screenshot, I have the feeling that sub-sub-section 1.1.1 content is not tagged as h4. When highlighting this text is it selected as h4 type?

The copy/paste from my example markdown content does not format h4 content properly (but rather as a simple paragraph). That behavior is a blocknote.js issue I guess; not directly related to this issue.
