docs: relocate INITIAL planning docs to PRPs/INITIAL and track optional-features (#197)

Summary

Documentation housekeeping (closes #197):

• Relocated all 20 root-level INITIAL*.md planning docs into PRPs/INITIAL/ via git mv (history preserved — diff shows R100 renames). Covers INITIAL-0.md..INITIAL-14.md incl. INITIAL-11A/B/C.md, plus INITIAL-EXAMPLE_EMPTY.md and INITIAL_EXAMPLE.md. Co-locates the discovery-phase docs with the PRPs they feed.
• Tracked the previously-untracked docs/optional-features/ directory — a README plus 9 candidate-feature briefs.
• Re-linked every documentation reference broken or made stale by the move.

The repo-visibility-audit skill was used to locate stale references; a re-scan reports zero broken internal INITIAL-*.md links.

Scope notes

• PRP *.md files contain only conceptual INITIAL-N mentions (no markdown links) — left untouched, nothing breaks.
• The moved INITIAL files have no outbound relative links, so the move breaks nothing inside them; their contents were not edited.
• .claude/ is gitignored in this repo, so the matching scope fix to the repo-visibility-audit skill is local-only and not part of this PR.
• A working-tree uv.lock change (self-version 0.2.8 → 0.2.13) was deliberately left out of this commit to keep the PR docs-only.

Acceptance

• All INITIAL*.md moved via git mv (history preserved)
• docs/optional-features/ tracked
• Zero broken internal INITIAL-*.md links
