In the midst of a diff review, I clicked on refine and asked GPT5 (high-reasoning) to amend a few things. However, I got throw an error below

I'm sorry, I couldn't complete that request.

Response stream finished unexpectedly with internal error: OpenAI API error: POST "https://api.openai.com/v1/responses": 400 Bad Request {

All subsequent prompts does not work regardless the model, gpt, sonnet, gemini...
