Include gen_ai.agent.name attribute in chat OpenTelemetry spans

Possibly related contribution from @c-ehrlich — opened #385 which appears to be addressing this.

Their summary: This PR propagates the agent name from invoke_agent spans down to chat <model name> spans as a gen_ai.agent.name attribute. This allows OpenTelemetry backends to associate chat spans with their originating agent directly, without needing to traverse the span hierarchy.
