Prevent the default block appender inserting duplicative default blocks

What problem does this address?

Default block appender creates duplicate paragraph blocks.

What is your proposed solution?

Match the behavior with the Notes app:

• If the last block is a paragraph, focus it and put the cursor at the end
• If the last block is something else, insert an empty paragraph block and focus on it

There was feedback shared (pbArwn-7kW-p2#comment-8516) regarding the default block appender. It was noted it's possible to tap beneath the invisible button and nothing occurs, which is confusing.
