create spreadsheet
create linked form with 2 fields
in the spreadsheets go tools>script editor
paste the code in the Code.gs window
go edit>current project triggers>addtrigger
Choose which function to run: respondToFormSubmit
Choose which deployment should run: Head
Select event source: From Spreadsheet
Select event type: On Form Submit
click save
go back to your code, select "respondToFormSubmit" from the "Select function" dropdown
press the play button
allow all access required
