Database connection reliability and diagnostics

• Explicitly enable SQLite URI handling when opening the local connection. This fixes a reproduced case where attaching the read-only history URI failed with SQLite CANTOPEN when the process-wide URI default was disabled.
• Bind the history filename as a statement parameter, including paths with spaces, quotes, percent signs, and non-ASCII characters.
• Wait up to one second for SQLite locks and reopen once after a transient read failure. Regular two-second refreshes continue automatically.
• Show the failed database filename and read stage in the error panel, with an awaiting-sync status during outages.
• Add Copy connection diagnostics to the error panel and settings. Reports include SQLite/OS error codes, file readability metadata, consecutive failure count, and last successful refresh time.
• Add an isolated regression covering URI defaults, special path characters, missing history files, and recovery after restoration.

Historical intermittent incidents lacked detailed error codes. The confirmed URI defect is repaired, and new diagnostic captures can identify any remaining causes.

Verification

The database regression and existing policy/language/layout checks pass locally. Twelve consecutive reads of live local data succeeded. The widget's active and unread counts matched the desktop app.

修复已复现的 SQLite 历史库 URI 打开失败问题，增加短暂失败重试和具体文件／阶段提示。错误面板及设置中新增“复制连接诊断”，可记录 SQLite 与系统错误码、文件可读性和最近成功时间，供后续排查。

历史截图的错误来源仍需新的诊断信息确认。本机已验证 12 次真实数据读取成功，任务与未读数量和 Codex 界面一致。
