Updates the ACTION triggering ubm udev rules to handle 'change' events in addition to 'add', since running dmap on F16/D16 was failing to generate vdev_id.conf.
Also merges in some previous hotfix commits pushed to build (HL4/8 slot order, missing F16/D16 Turbo stuff in server_identifier).
