Minor Aztech changes

Summary

Two minor changes related to the Aztech cards:

• Use the WSS mixer for CD audio and OPL3 when in WSS mode on the Aztech 2316 (Washington) cards, fixes the CD and FM volume controls in the Windows drivers.
• Add support for port 630h to the Panasonic/MKE CD-ROM interface as this is used by Packard Bell's Forte16 (Nova16/AZT1605) sound card CD-ROM interface.
