For as long as I can remember, whenever you use a function in the tool that duplicates any entity, if the entity is unable to be duplicated successfully, it will break the duplicating part of the function until you relog.
Should be pretty easy to reproduce, just set low limits and hit them using any function that can duplicate an entity.
