Remove stale Claude pre-commit hook

What

• Delete the tracked .claude/hooks/pre-commit-guard.sh local hook.
• Remove the matching unreleased changelog bullet.

Why

The repository should not carry Claude-local commit/stash/push guard logic as project source. Removing the changelog bullet keeps the unreleased notes honest.

Validation

Not run locally. This PR is intentionally small and ready so GitHub PR workflows, including Claude Code Review, trigger on a real cleanup change.
