Wrong initial evaluator name is shown on create

In v0.100.1, the evaluator name appears to be memoized and not properly reset.

After creating a prompt, creating an exact match evaluator shows the wrong initial name. Creating a judge afterwards also shows the wrong initial name.

This is reproducible in local, Railway, and preview.
