🎨 Palette: Add visual indicator to recent history items

💡 What: The UX enhancement added a visual indicator to history items.
🎯 Why: The user problem it solves is that previously, when DeepLens was opened with no query, it showed recent files/symbols but didn't clearly demarcate that they were recent history rather than default suggestions, except for the window title.
📸 Before/After: Visual change in QuickPick dropdown, prepends $(history) Recent • to the item descriptions.
♿ Accessibility: The screen reader will now read "history Recent" before the description of historical items, adding clear context.
