FAILURE ATLAS is a cross-domain map of structural gaps that civilization keeps falling through — named, sealed, and made undeniable.
It is not a research paper. It is not a framework collection. It is not a critique.
It is a cartography of forgetting. Every domain has failure nodes it refuses to look at directly. Every era inherits unsolved problems from the previous one and rebuilds them from scratch without reading the autopsy. The solutions that already exist go uncarried across disciplines. The same crack appears in different materials across centuries and nobody draws the full building.
This repo draws the full building.
Every entry has four bones:
- The Gap — what doesn't exist that should
- The Cascade — what falls when the gap stays open
- The Human Consequence — the specific person, not the statistic
- The Date Named — sealed permanently via STP FROZEN-2.0

Any single floor is ignorable. A building with nine floors showing the same structural crack in different materials is not.
[R] The solution to most named gaps already exists — in a different domain. The Roman aqueduct solved Hyperloop's thermal expansion. The prairie dog solved seismic exit architecture. The Gothic cathedral solved acoustic resonance in stone chambers 600 years before the problem was created again in steel vacuum tubes.
FAILURE ATLAS maps not just the gap — but where the answer already lives that nobody carried across.
Three bones beneath the four entry bones:
Bone 1 — Domain Blindness. Every field has a failure node it refuses to look at directly because looking means admitting the field didn't solve it.
Bone 2 — The Adjacency Problem. The solution exists. In a different domain. Uncarried.
Bone 3 — The Consequence Chain. A specific human at the bottom of every gap. Not a statistic. A moment. A failure already named and still nobody moved.
The most diabolical floor. It doesn't document new failures. It documents the same failure repeating across centuries with different materials.

This Atlas is read-first. Pull requests are disabled. The architecture is not open for external modification.

If you have found an entry that is factually wrong — a gap that has already been closed, an ancient solution that has been misattributed, a human consequence that is inaccurate — contact directly:

Include the floor, the room ID, the specific claim, and your evidence. If the challenge holds under scrutiny — the entry gets corrected and the correction is sealed and logged permanently.
