Enable plugins to alter the generated markup

I am working on some Mark Down extensions to make it easier to create tables. It would be handy to allow a plugin to be inserted that could handle these extensions.

I have already written the code to handle this. So if you are interested in this change, I can submit a pull request to get it in.

Submit a pull request and we can have a look and discuss :) it may be better to add it upstream at the markdown library level but happy to look :)
