Add 15 tests covering all branches of _generate_steps() in migration.py: - path_removed, operation_removed, parameter_became_required - required parameter_added, schema_removed, type changes (property/schema/parameter) - property_removed, enum_values_removed, request_body_became_required - request/response content_type_removed - unrecognized breaking change fallback, combined multi-step scenario Add 1 test for loader.py: unknown extension where both YAML and JSON parsing fail (previously untested fallback error path). All 155 tests pass (up from 143).

Adds 16 new tests covering previously untested branches.
Migration module (15 tests): All step-generation branches in _generate_steps() now have dedicated test coverage including path_removed, operation_removed, parameter_became_required, required parameter added, schema_removed, type changes, property_removed, enum_values_removed, request_body_became_required, content_type_removed, fallback step, and combined scenario.
Loader module (1 test): Unknown extension where both YAML and JSON parsing fail.
All 155 tests pass (up from 143). Low risk - test-only PR.
