Two unrelated additions previously carried as untracked WIP:
- **docs/claude/retrofit.md** — playbook for retrofitting apps that
pre-date ADR-003's `@spec` annotation convention, via the ghost-change
pattern. Referenced by /opsx-coverage-scan, /opsx-annotate,
/opsx-reverse-spec.
- **docs/iso/{README,code}.md** — first slice of an ISO compliance
initiative. code.md maps every clause of ISO 9001:2015 (with the
ISO/IEC 90003:2018 software interpretation) to concrete artefacts
across the four pipeline layers (Specter / Hydra / Quality CI /
human reviewer), with explicit gap calls per clause (Full / Partial /
None / N/A).

Took origin/main's expanded retrofit playbook (from PR #36): updated title, ghost changes section, relative skill links, procest as testbed example, Bucket 2 un-spec'd methods table, and revised roll-out order. Co-Authored-By: Claude Sonnet 4.6 <noreply@anthropic.com>
