- Reading the note a guard block demanded is no longer punished (#148): the guard records the demanded note in a per-turn marker; the verifier treats a consult of it as relevant by definition. Kills the self-contradiction where the verifier re-closed the gate over reads of the exact note the guard's repo-work rule requires. An undemanded read is still judged on its merits.
- Vault writes are no longer scored as consults (#148):
create/edit/delete/restore-noteno longer clear the consult-gate at PreToolUse nor get relevance-scored at PostToolUse — both bash adapters gate them like ordinary actions. The Hermes adapter also gains the navigation-tool exclusion the Claude adapter already had. Note: the adapter half lives in the installed hook scripts and activates when hooks are reprovisioned.

off-topic-consultevents carry adetailfield with the deterministic relevance score and the task/activity/pending signals the consult was judged against. Verifier thresholds remain untouched pending a sampling pass over the enriched log.
