Context

Need to update the content in the cancellation form and preview page to reflect the changes in the Notify template.

Changes proposed in this pull request

Update the content in the letter partial which will update both the form and preview pages.

Guidance to review

  1. Visit a placement request
  2. Click 'Reject request'. The form should have the new content
  3. Fill out the form and click preview. The preview page should display the new content
