[DOCS]: Bug report template lacks structured reproduction and context fields

The existing bug report template is too generic and does not guide users to provide consistent reproduction details.

Current Behavior

Bug reports often miss environment info, actual vs expected behavior, and helpful context.

Why This Improvement Is Needed

A more structured bug template helps contributors provide better issue data and simplifies triage.

Proposed Solution

Update the bug report template to include:

• Summary
• Steps to reproduce
• Actual behavior
• Expected behavior
• Environment details
• Screenshots/logs
• Proposed fix

Expected Outcome

Bug reports become easier to reproduce and resolve, improving overall project quality.
