Refactor core proxy infrastructure

• Merge grpc servers (currently there's 4)
• Rethink and simplify authentication for each server
• Rethink and simplify tokens used in proxy-client communication (introduce token types?)
• Decompose and simplify start_enrollment grpc endpoint
