🔧 Cross-platform Stability (#7)

• PATH inheritance from process.env.PATH instead of hard-coded macOS paths
• CLAUDE_BIN env var override for custom claude binary locations
• proc.unref() to prevent parent process blocking on detached child
• Ready event listens for CLI init signal instead of blind 2s timeout
• contextPercent calculation fixed (was hard-coded 0)
• resumeSessionId exposed in tool schema

⚡ Lazy Init (#8)

• SessionManager + EmbeddedServer deferred to first tool call
• Zero memory overhead when plugin is installed but unused
• claude_session_list returns [] immediately if manager not initialized
