v2.9.2 — Race Condition, Streaming Timeout, Error Handling

Fixed

• Session creation race condition — concurrent startSession() calls now properly deduplicate
• Streaming proxy timeout — streaming handler uses fetchWithRetry instead of bare fetch
• 7 swallowed errors — empty catch {} blocks in PersistentClaudeSession now log via SESSION_EVENT.LOG; process kill catches distinguish ESRCH from EPERM
• Hook errors logged — _fireHook no longer silently swallows callback errors
• Unsafe type casts removed — as unknown as double casts replaced with proper validation + typed fields
• max_tokens validation — OpenAI-compat endpoint rejects non-positive values with 400

This completes all findings from the v2.9.0 code quality audit. No remaining known issues.
