Scope:

• Add src/hooks/useTransactionThread.ts
• Add src/hooks/useReportActionsPagination.ts
• Update ReportActionsView.tsx
• Update useLoadReportActions.ts
• Expose isLoaded from useTransactionsAndViolationsForReport.ts if needed

Risk: Medium. Touches pagination and thread stitching.
