Recover cleanup identity from stored metadata

Summary

• Recover blank cleanup/reconciliation repo, branch, and path identity fields from trusted stored worktree metadata when it matches the handle/path identity.
• Classify detached and protected/base-branch worktree cases with specific reason codes, hints, and stored identity evidence instead of collapsing into generic missing/protected metadata buckets.
• Add targeted smoke coverage for detached stored-branch recovery, protected-base diagnostics, and inconsistent identity metadata blocking.
