Thanks for this great tool! feel very clean and straightforward.

I am using VSCode + Github Copilot + Claude Sonnet 4.5, seems that some times the tasks.md is not respected when execute openspec-apply.

for example for a change which adds a new Widget (UI component) to the application, I would like the document widget-library.md which documents list of widget also being updated automatically, it did add a task in tasks.md, but after the apply finished it just mark the task done but in fact the document was not updated at all.
