- rename request-context types and prompt-builder APIs to user-context for clearer semantics
- split prepended tool reminders into skill listing, agent listing, and collapsed tool listing
- update execution prompt injection to preserve ordered multi-reminder delivery
- align tool guidance, agent policies, tests, and prompt wording with the new naming model
