Drop `miss_index` if it is not needed to indentifiy rows with differences

Summary and report output includes `miss_index` when showing the diffs for a particular column, but these may not be needed. They will certainly not be necessary if there are unique IDs that were used and that are added, it may be useful if the IDs are not unique or if the comparison was done by position.
