feat(tui): replace Flow Map with Agent Discussion Panel

Purpose

Remove the existing Flow Map TUI component and replace it with an Agent Discussion Panel that shows real-time agent stances, cross-reviews, and consensus building.

Changes

• apps/mcp-server/src/tui/ — Remove Flow Map component
• apps/mcp-server/src/tui/ — New AgentDiscussionPanel component (Ink/React)
• Real-time updates as specialists report via Teams SendMessage

Acceptance Criteria

• Flow Map component removed
• Agent Discussion Panel shows: agent name, color, stance per round
• Cross-review arrows displayed
• Consensus meter (X/N, Critical count)
• Scrollable discussion history
• Updates in real-time as opinions arrive
• Can be merged independently
