The /prism skill was confusing since users should never invoke it directly.
Its content was ~90% redundant with the CLAUDE.md template. Relocates the
unique pieces (routing table, artifact evidence) to the template, updates
skill cross-references to point at project root CLAUDE.md, and fixes the
cli.py fallback string.

- Fix success_criteria to use structured SuccessCriterion objects (claim/output/condition)
  instead of plain strings which would fail validation
- Document conditional decisions (when: field) with example
- Document constraint types (incompatible_with, requires) with example
- Document excluded/excluded_reason on options

Collapse multi-stage example into a one-line summary, trim insights to
one evidence type, compress recipe and CLI sections, remove the when
example's redundant parent decision.

CLAUDE.md is already loaded into the system prompt for every conversation
in the project — skills don't need to reference it separately.
